Outreach Service for Older People within the Community

For those who cannot get into the centre, Mid-Lin does offer an outreach service to service users.

We currently have two outreach workers, so the places available on outreach are more limited than the number of places available within the day centre itself.

What Is Mid-Lin’s Outreach service?

Mid-Lin provides an Outreach Service for older people to alleviate social isolation and loneliness.

Mid-Lin aims to help and support the service users to be part of their wider community.

Our aims and objectives are:

  • Independence

  • Dignity

  • Privacy

  • Choice

  • Safety

  • Motivation

  • To promote social stimulation

  • Prevent isolation

  • To promote and sustain daily living skills

  • To contribute to rehabilitation

  • To work alongside other health professionals to promote independent living in the community

  • To provide respite for carers

  • Engagement with healthy living initiatives.

 Mid-Lin Day Centre Outreach Service

The Outreach Service provides short term respite for carers and  social support for the service user at home. It gives opportunities for social interaction and stimulation, as well as in your local community.

The Outreach Service is available Monday to Friday and is run by a team of staff and volunteers.  All of the team are members of the Disclosure Scotland PVG Scheme.                                   

 Activities:

A wide range of social activities are on offer based on the service users’ likes, dislikes and feedback.

These include:

  • Support to go for lunch or coffee and a cake.

  • Quizzes and games

  • Life stories and reminiscence

  • Board Games

  • Dominoes

  • Going for a walk.

 How much does the Outreach Service cost?

The Outreach Service currently costs £20.00 per hour, which is subject to annual review.

Referrals for Mid-Lin Day Centres Outreach service

Referrals can be made by individual’s or family members as well as by carers and health care professionals.

Following your initial referral, you will receive an offer to meet our Outreach worker and discuss what we can offer.
